 Please register before registration deadline Age Groups and divisions fill fast and we may have to close out age groups before the registration Deadline

 After you mail in registration form and your team is accepted. We will be mailing you a packet of information.

Hotel Information

Forms that need to be filled out and mail back

- One Copy of the Player Roster (Form A) / please fill out and mail back

- One Copy of the Guest Player Form (Form B) / please fill out and mail back

- One Copy of the Permission to Travel form approved by your state soccer association / Out of State Teams Only / please fill out and mail back

- One Copy of The Emergency Medical Release & Liability Wavier / please fill out and bring to the tournament

 What you need to be bring to the Tournament

- Two Copies of the Player Roster (Form A)

- Two Copies of the Guest Player Form (Form B)

- One Copy of Your State Roster

- One Copy of The Emergency Medical Release & Liability Wavier for each player that will be participating in the tournament (Form C)

- Player Passes with a picture ID for each player that will be participating in the tournament

- Coaches and representatives are required to be at the registration tent One Hour prior to the first scheduled game.

 Important Notes:

- Players Emergency Medical Release & Liability Waivers will be kept by the tournament officials and will not be returned after the tournament.

- Players having to wear glasses while they are playing must wear sport goggles

- Due to increasing number of teams registering there will be a possibility of some friday evening games.
